1. Medical Services and No Emergency Care

3. Patient Responsibilities
By receiving services from PrevHealth, you agree to provide accurate, current, and complete information about yourself, including medical history and contact information. You are responsible for updating your information as necessary to ensure continuity of care and accurate billing.
You agree to comply with clinic policies, appointment procedures, and payment obligations. Failure to provide accurate information may impact care or billing processes.
4. AI-Assisted Documentation
PrevHealth utilizes AI-assisted documentation tools to support clinical workflow efficiency and administrative functions. These tools assist providers in organizing information and drafting documentation but do not replace clinical judgment. All diagnoses, treatment plans, prescriptions, and medical decisions are made by licensed healthcare professionals.
By using our services, you acknowledge that AI tools may be used in documentation support but that medical decision-making remains the responsibility of your provider.
5. Payments and Billing
PrevHealth processes payments through third-party payment processors such as Stripe and Square. PrevHealth does not store full credit card numbers. By submitting payment information, you authorize PrevHealth to charge the applicable fees for services rendered, including co-payments, deductibles, self-pay balances, and other patient financial responsibilities.
You acknowledge that insurance coverage verification does not guarantee payment of claims and that you remain responsible for amounts not covered by insurance. Refunds, if applicable, will be handled in accordance with PrevHealth's billing policies and applicable law.
6. Mobile Messaging Terms
If you provide your phone number and consent to receive text messages, you may receive SMS communications related to appointment reminders, registration links, intake forms, billing notifications, payment links, verification codes, and care coordination communications.
Message frequency varies depending on your interaction with our services. Message and data rates may apply. You may reply STOP at any time to unsubscribe from SMS communications. You may reply HELP for assistance. Opting out of SMS communications does not affect your ability to receive medical care, though alternative communication methods may be required.
PrevHealth does not sell or share your phone number with third parties for marketing purposes.
7. Acceptable Use of Website and Systems
